Each vendor/concession stand will be responsible to acquire a health and transient vendor permit if food is being prepared. This year we need a copy of your permit. If you need the forms contact Tracy Skelton at 330-XXX-XXXX. All Civic Organization/Club or Church must send the form to the Stark County Health Dept. A.S.A.P. It is estimated that 12,000 to 15,000 people attend this event.
A letter will be sent prior to the 2010 Lift Off with your space location. You may set-up on Thursday. There will be someone to direct you to your space on those days. Please come in through the main entrance.
The City of Louisville will make electric & water available for your use. It will be your responsibility to furnish all extension cords, hoses and fittings for your connection. Please reserve one space for our Vendor Co./Restaurant/Service Group/Church at the Louisville Constitution Lift-Off on Friday, August 21 & Saturday, August 22, 2010. This form is to be filled out by non-food vendors also.
