Vendor/Concessionaire Application

 

The 2009 Louisville Constitution Balloon Lift-off Festival will be held on Friday, August 21 & Saturday, August 22.  Below is an application to participate in this exciting weekend as a vendor or concessionaire.

 

Each vendor/concession stand will be responsible to acquire a health and transient vendor permit if food is being prepared. This year we need a copy of your permit. If you need the forms contact Tracy Skelton at 330-875-1908.  All Civic Organization/Club or Church must send the form to the Stark County Health Dept. A.S.A.P.  It is estimated that 12,000 to 15,000 people attend this event.

 

A letter will be sent prior to the 2009 Lift Off with your space location.  You may set-up on Thursday. There will be someone to direct you to your space on those days. Please come in through the main entrance. 

 

The City of Louisville will make electric & water available for your use. It will be your responsibility to furnish all extension cords, hoses and fittings for your connection. We will have ice available for vendors to purchase at $1.00 per bag.   For further information contact Tracy Skelton at 330-875-1908.

August 21 & 22, 2009

Please reserve one space for our Vendor Co./Restaurant/Service Group/Church at the Louisville Constitution Lift-Off on Friday, August 21 & Saturday, August 22, 2009. This form is to be filled out by non-food vendors also.

 

Mark which space rental is needed:

We understand that this application becomes a contract when signed by us and accepted by the Louisville Constitution Lift-Off Festival Committee. We agree to abide by the regulations governing this event as printed herewith. It will be the vendor’s responsibility to acquire a health and transient vendor permit if food is being prepared.
